With the exception of the Introduction and chapter 4, the chapters in this volume have appeared in similar form as essays in journals and edited collections over the past decade or so. They have all been reframed and rewritten in order to make clear their relation to one another and to eliminate redundancy, though with few exceptions I have not attempted to revise their arguments or augment their bibliographies. I am grateful to the publishers of the journals and books in which these essays appeared for allowing me to rehearse the ideas in this book first in their pages.
